Innis & Livonia Community Health Centers – innishealth
Innis Community Health Center, Inc. began in 2001 providing affordable primary care health services to the communities of Pointe Coupee, southern Avoyelles parish and northern Ibervile parish. Innis achieved the status of being a federally qualified community healthcenter by receiving HRSA 330 Grant funding to operate as a FQHC. The services of this comprehensive community health center include providing medical outpatient services, dental care to adults and children and behavioral health care services. INCHC is a not-for- profit health center governed by a local Community Board of Directors.
To Provide Primary Healthcare Services to Area Communities in Need of Coordinated, Affordable, Preventative Health Care in a Prudent, Efficient, and Caring Manner.
Through community collaboration and partnership, the Board of Trustees and staff of the Livonia and Innis Community Health Care Centers will develop and promote supportive health care services to all people who are medically under-served, in order that they may experience all the rights, privileges, and responsibilities as a member of this community. In addition, we will assist the communities of Pointe Coupee, Southern Avoyelles and Northern Iberville parishes to achieve their greatest potential and to live healthy and purposeful lives. We will provide health education, family support, and community resources in assisting them to take responsibility for their health status. Our strategic vision is to be your healthcare home of choice.
Innis CHC has grown over the past 15 yrs in its programs and locations to serve more communities. Primary Care is its focus of healthcare delivery. Primary care sites include Livonia Health Center, Innis Health Center and Maringouin Health Center.
These sites provide:
- Routine primary medical care- family practice
- Health screenings for blood pressure, vision, hearing, diabetes
- Medication Assistance programs
- Chronic Disease Management for Diabetes, Hypertension and other forms of chronic diseases.
- Health Education and Nutrition education.
- Mental health services
- Dental services for children & adults to include: cleanings, restorative work and prevention sealants
- Podiatric services
- Sports Physicals
In addition the organization entered into the school based health clinics in 2007 and now have 2 sites for those health care delivery services to the children of Pointe Coupee parish. Locations include a site which services the 3 schools in the central and northern part of the parish and an additional site supporting 3 schools in the southern part of the parish.
The Dental Program has also expanded to include a Fixed based dental clinic located in Innis and a Mobile Dental Clinic supporting the schools.
