Innis Community Health Center Inc., as a federally qualified community health center, offers services at a discounted price to those who qualify.

In addition the health centers accept all major insurances to include those governmental sponsored programs through Medicare and LA Medicaid (known as Healthy Louisiana). The clinic files claims to specified insurance agencies.
Co-payments /sliding fees are collected on site at the clinics and are expected to be paid at the time services are rendered. For patients with Dental insurance the clinic may assist the patient in filing claims.

Innis Community Health Center Inc. offers a sliding fee discount to all uninsured and underinsured individuals who qualify based on an approved sliding fee scale. This sliding fee scale provides a variable cost on certain service to individual that qualify. This sliding fee scale is based on the United States Department of Health and Human Services (HHS) Federal Poverty Guidelines and uses household size and income to determine eligibility. The Federal Poverty guidelines assists INCHC in assessing eligibility for individuals living at and/or below Federal Poverty Level.

In order to apply for a sliding fee discount, the patient must provide household size and proof of total household income. Gross annual income is all income from all sources before taxes. This is required at the time of the visit to qualify for sliding fee scale. The Statement of Income declaration is required to qualify. Once eligibility is established, the patient’s charges will be discounted down to a minimum fee. All minimum fees are due at the time of service. Proof of income must be updated annually.

All patients must submit proof of income each year in order to continue receiving discounted services.
